Member Fees:

• Enrollment: $150

• 5% Discount for 2+ individuals from the same household

• Children 0-17 years old: $35/month with a paying adult

• Adult 18-49 years old: $60/month

• Adult 50-64 years old: $75/month

• Adult 65+ years old: $80/month

• Family Maximum: $250/month

Non-Member Self-Pay:

• New Patient Visit: $200

• Established Visit: $150

• New Patient Wellness: $250

• Established Patient Wellness: $200
